Referral from your doctor: Your insurance carrier may require that your Primary Care Physician complete a referral form to authorize radiology services. The referral form authorizes payment to the participating radiology facility. Please bring your referral form with you at the time of your appointment.
Pre-Authorization: Some insurance carriers require your referring physician to obtain a pre-authorization number for select radiology procedures such as MRI’s, and CAT Scans. The pre-authorization number authorizes payment to the participating radiology facility. Please bring your pre-authorization number with you at the time of your appointment.
My insurance coverage: Every plan is different. While most plans, now, cover radiology services there are still some that do not cover 100%. Deductibles and co-pay policies have recently changed with some carriers. We advise you to contact your individual insurance company prior to the procedure if there are any questions regarding coverage.
Co-Payment: Managed care co-payments will be requested at the time of service and may be cash, personal check or major credit card. Co-payments are a part of the contractual agreement between RAH and your insurance carrier and cannot be written off or adjusted.
RAH “Self-Pay” policy: Payment is requested at the time of service and may be cash, personal check or major credit card. If you are unable to make payment in full, then arrangements for settlement of the account will be agreed upon and documentation will be sent to a representative in the billing office.
Major credit cards accepted: We are happy to process payment using Mastercard or Visa. We process payments electronically and have a secured system for the safety of our patient’s personal identity.
